A birth, death or marriage certificate is an official document containing registered information about a person's birth, death or marriage. Anyone can purchase a certificate by providing the details we need and paying the fee of NZ$26.00. You can provide these details by phone, letter, email or fax, or by downloading, completing and sending our Request for Birth, Death or Marriage Certificate Form (.pdf) 28k.
